Dear Patient,

West Central Manchester Primary Care Network in partnership with Manchester Local Care Organisation (MLCO) and the Caribbean & African Health Network (CAHN) are conducting a number of focus groups to identify the experiences of people living with Type 2 diabetes in accessing Primary Care diabetic appointments, with a view to improving and addressing inequalities in service provision.

We would like to invite you to participate in this project because as someone living with Type 2 Diabetes your experiences are extremely valuable, and your contribution will help to inform future practice and shape how local health services gather feedback from patients.

The focus groups for adult patients living with Type 2 diabetes will take place on:

Thursday 16 February from 4.30pm to 6.30pm
Friday 17 February from 4.30pm to 6.30pm

Sessions will be delivered by the Caribbean and African Health Network (CAHN) and venue information will shared when expressions of interest have been received.
